EOS Easter Orchestra Week Application Form
Please use this form if you are a string player, or if you are a brass/woodwind player who has played with us before. If you are a woodwind or brass player and have not been to the Easter Week before, we ask you to apply first to the Waiting List (please see Home page for the link). We will contact you if a vacancy becomes available. You do not need to pay a deposit to join the Waiting List. Please note that questions marked with a star '*' require a response.
All subscriptions are per person.

Please note that for Gift Aid to apply, you need to pay the same amount or more of UK Income Tax and/or Capital Gains Tax as all charities will claim on your donations in a tax year and you are responsible for paying any difference.

The deposit is non-refundable if you decide not to attend the week. In the event that the event has to be cancelled (e.g. because of Covid restrictions), we will refund all deposits and subscriptions.
